Since you're on this page, you've likely tried to pull up our new patient forms on GoFormz and had issues.

Let's make sure you've followed the rules we gave you.

• Did you try to pull this up on a phone? It's not formatted for viewing on a phone. Please don't.

Our next question we have for you is:

• Do you have a printer or access to one? If you don't, downloading the documents won't help you. You'll need a printer to print out the >10 pages so you can fill them out by hand. If you do have a printer, click here for the adult form or click here for the pediatric form (under 14 yr old)

If you don't have access to a printer and can't use the GoFormz online forms and there's time before your appointment, come by the office and we'll give you a paper copy of our forms that you can fill out in the privacy of your home.

If you've procrastinated filling out your forms and there's no time (or we're closed) to get a paper copy, then the only solution we have for you is to have you come in at least 1 hour prior to your appointment so that you can fill out your forms at the clinic.

If no digital forms, no printer, and you really can't get here before your appointment, then you should probably call and cancel your appointment until we can get your paperwork situation squared away.
