If your printer ran out of ink or isn't working, not to worry. You're always welcome to pick up the paperwork from us or stop in 40 minutes early to fill it out before your visit. (IMPORTANT: You will need to have your paperwork completed before Dr. Berglund can see you! Please be sure to plan ahead and have it filled out!)

Insurance and payment options
Most of our patients take advantage of our "50% same day cash" discount. This means that if you pay for your services on the same day that they are rendered (and we don't submit any claims to your insurance company), we automatically take 50% off of your office visit charge. (This does not apply to lab work, orthotics, supplements, or retail items.) Since Dr. Berglund is an out-of-network provider for all types of insurance, this helps a great deal when it comes to your health care costs at our clinic. If you DO decide that you want us to submit your claim to your insurance, we will need to verify your coverage ahead of time. Please contact our clinic so that we can get your insurance verified before your first visit. Our "disclaimer" when it comes to billing insurance is that coverage is never guaranteed, and if your insurance company denies any or all of your claims, you will be responsible for that amount. (We do not accept WEA or Anthem Blue Cross Blue Shield. We have found that when we submit claims to them, they pay much less than what our 50% discount offers.)

If you have insurance, yet you still want to take advantage of our 50% discount, you are welcome to submit your claims to your insurance company on your own. How does this work? After your office visits (and after your bill is paid, in order to get the 50% discount), we can print a receipt for you which includes diagnosis codes, procedure codes, and what you were charged. You will then need to contact your insurance company and ask them to send you HCFA forms. Some insurance companies even allow you to download them from their website.) Once you have the forms, simply transfer the information from your receipt to that form, and send it in to your insurance. Once your deductible has been met, you should start to be reimbursed by your insurance company!

If you decide to submit your claims on your own (after taking advantage of our 50% discount), we are legally not allowed to help you out with this process in any way. We also cannot guarantee that your insurance company will cover your claims. However, a lot of our patients who have given this a try have actually done quite well with being reimbursed!
